We have exploded with excitement at DreamLIfe Destiny from the powerful community we are creating. Our dual services approach with community integration and self advocacy has captured the imagination of our clients and the love and thanks of numerous caregivers and case managers across the metro area. Our goal is to assist individuals with creating their own dream life for a better destiny through weekly supports, creative opportunities, natural supports, friendship building, and opportunities that they would never have had otherwise.

Our goal is to help you live better, dream about your future, and assist you in making your destiny come alive. Tyler Segraves and Russell Peterson, owners of DreamLife Destiny, have developed a unique community oriented and individual focused system that meets all 245D requirements in Minnesota and goes beyond Person Centeredness to build trusting relationships, incorporate effective teaching moments, enhance emotional support, and reach for the future.

Our clients are engaged in their lives beyond doing daily tasks or living healthfully. The individuals we support also enjoying special events, classes, networking, and community engagement. We facilitate independence, strengthen unique abilities, find connectedness, and promote happiness!

DreamLife Destiny offers a dual program in Individualized Home Supports or IHS with Training and 24 Hour Emergency Services or EA. We meet all the requirements of the State of Minnesota for IHS with Training and have a unique training program for our staff that not only lays foundational knowledge with a person focused approach, but incorporates trust building, community integration, and creativity. Additionally, we offer opportunities for community building events and travel that works within your budget and goals. We have a regular Friday Coffee Club, frequent Art Club, Fitness Club, Recovery Club, Activity Days, Nature Outings, and Gaming Nights. We do special events and seasonal offerings throughout the year working to get individuals into the community and meet new friends.

Our 24 Hour Emergency Assistance Phone Line integrates weekly mental health check-ins and regular monitoring of critical situations. We do this to fully understand our clients, help promote independence, assist with problem solving, give families and caregivers a break, and provide better assistance when a crisis develops. You can call this line 24/7 and be assured that the person assisting you understands your situation and will help you in an Emergency Situation.

The IHS with Training Remote Service targets your specific needs to keep you on track weekly as well as provides problem solving and processing of issues or concerns you may have. This gives you greater consistency for in person services. Emergency Respite is a service that we offer on an as needed basis. We negotiate the terms and conditions directly with your case manager so that it reflects what you need at the time. We also provide 245D Consulting to programs who need to improve services and compliance.

We work in the Minneapolis and Saint Paul metro area serving much of the following counties including Anoka, Dakota, Hennepin, Ramsey, Sherburne, and Wright. Other areas by individual determination. We take referrals from county agencies, social service companies, social workers, case managers, parents, family, and friends as well as self referral from individuals. 

We work with waiver services and private pay as well. Please make referrals on our referral page. This is the best way for us to understand from a preliminary perspective how we would need to set up services and make follow-up connections.

We will co-ordinate with the referral team, do a follow-up screening, and have an initial in-person visit to determine a perfect fit and placement with one of our Destiny Associates.
